Joining our 500 corporate participants to
transport cancer patients to life-saving, state-of-the-art cancer
treatment is a simple process of filling out a registration

How does Corporate Angel
Network's program work?
Once you have joined Corporate Angel Network,
we obtain your flight schedule on a weekly basis by fax, phone
or email, whichever is most convenient for you. Your schedule
information is entered onto the database on a secure server;
then all paperwork is immediately shredded for your security.
When a patient requests transportation and our database indicates
you have a scheduled flight within 100 miles of both departure
and destination airports, Corporate Angel Network will call
to verify whether the flight is going and seats are available.
If seats are available and the patient can take that flight,
Corporate Angel Network calls you back to request the flight
and obtain executive clearance. We fax you a completed flight
sheet to be used for your manifest, including the patient’s
name and all contact information, along with ground transportation
arrangements.
The patient is then given the details of the flight. the company,
airport, hangar name and phone number for directions, departure
and arrival times. Corporate Angel Network requires the patient
to arrive at the hangar one hour before departure. The patient
is advised about appropriate dress (no jeans or sweats), luggage
and ID requirements, and behavior (no smoking or drinking of
alcohol). In case of last-minute flight cancellation, simply
notify us and we will advise the patient. Conversely, we will
alert you should the patient have a change in plans.

Since
our flight schedule changes all the time, how can we accommodate
the patients?
All participating
companies have frequent changes in their flight schedules. Because
schedule changes are the nature of corporate aircraft operations,
the patients book back-up commercial flights so they can still
get to their treatment in the event of a cancellation.

| 4. |
Is additional insurance
required?
Patients are covered for liability by your
already-existing insurance policy. However, if you wish, patients
can sign a release form provided by Corporate Angel Network,
which will be faxed to your flight department prior to a flight.

| 5. |
Are there any tax ramifications?
No. The IRS has confirmed that this is
not a taxable event for the patient, and there is no additional
tax benefit to the corporation. The company is not incurring
any additional expense since the staff travels in the normal
course of company business.

What if a flight must
leave ahead of schedule? Will we have to wait for the patient?

departure.

How ill are the patients?
(There are no medical facilities onboard.)
A. Patients arrive suitably dressed for business
travel one hour before departure. They can climb the steps to
your plane unassisted and require no medical assistance onboard.
This is verified with their physicians before we contact you.
Corporate Angel Network verifies with every one of its patients’
physicians that each is healthy enough to comfortably travel,
can walk unassisted onto the aircraft, and do not need any assistance
on board.
